The Family Caregiver Support position plays a vital role in strengthening and supporting caregivers by working with them—not for them. Through direct support, community collaboration, and thoughtful care coordination, the Family Caregiver Support person ensures caregivers feel informed, supported, and capable as they navigate their caregiving journey.

• Ensures full compliance with state and federal requirements for the National Family Caregiver (FCG) and Older Relative Caregiver (ORC) Support Programs to maintain program integrity and funding eligibility.
• Obtains Access Care Coordinator endorsement and attends mandatory trainings to fulfill mandatory twelve credit requirement.
• Contributes to annual planning and budget development to support sustainable and effective service delivery.
• Supports ongoing program development by identifying needs, improving processes, and aligning services with organizational goals.
• Produces accurate and timely monthly service unit reports to support oversight, evaluation, and funding requirements.

• Delivers individualized, one-on-one support to family caregivers by assessing needs, monitoring progress, and promoting caregiver stability and well-being.
• Provides comprehensive direct services that result in informed decision-making and improved care outcomes, including:
• Information and referral to appropriate resources.
• Needs assessment and service planning.
• Care plan development, implementation, and follow-through.
• Crisis intervention and problem resolution.
• Outreach to underserved or at-risk caregivers.
• Case oversight and quality assurance.
• Periodically conducts home visits to assess caregiver environments, strengthen engagement, and ensure services are responsive to real-world need.
• Assists with maintaining complete, accurate, and timely client records and encounter notes to ensure compliance, support quality service delivery, and meet reporting requirements.

• Builds and maintains collaborative relationships with community agencies and professionals to expand service access and reduce service gaps.
• Communicates timely, relevant information affecting seniors and caregivers through local media and public outreach to increase awareness and utilization of services.

• Guides care partners through future care planning.
• Leverages community and organizational resources to connect caregivers with timely, appropriate supports.
• Serves as a key liaison between Senior Citizens Council and partner organizations to strengthen coordination and continuity of care.
• Coordinates and facilitates meetings to ensure effective communication and delivery of services across stakeholders.

• Communicates clearly and professionally with clients, staff, volunteers, and community partners to support trust, coordination, and effective service delivery.
• Develops and distributes Family Caregiver (FCG) and Older Relative Caregiver (ORC) newsletters to inform stakeholders and promote program engagement.
• Demonstrates strong written and verbal communication skills to support documentation, training, outreach, and collaboration.
• Utilizes public speaking skills to educate, inform, and engage caregivers, partners, and the broader community.
