Responsible for planning, coordinating, providing and documenting care for an assigned caseload of patients, which may include nursing care to terminally ill hospice patients as needed. Provides assistance and understanding to the family in the home care situation, and in time of bereavement. Works as a member of the hospice team in providing hospice care. Utilizing physicians’ orders and professional skills, the Home Care/Hospice RN develops and implements a plan that meets each patient’s specific needs and complies with JCAHO, federal and state regulations, reimbursement guidelines and agency policies and procedures.
Participates in performance improvement and quality assurance activities.
- Develop a written plan of care for each patient.
- Provide skilled nursing care.
- Assess patient’s condition, initiate plan of care, re‑evaluate and update as necessary.
- Assess and re‑assess pain; utilize appropriate pain management techniques; educate patient and family regarding pain management.
- Assist in developing pain and symptom management plans; re‑assess plan to keep patient comfortable; notify primary care physician when pain not manageable.
- Represent the organization positively and professionally.
- Participate in continuous quality improvement and performance improvement activities.
- Perform all aspects of patient care in an environment that optimizes patient safety.
- Complete assignments accurately and on time.
- Obtain all necessary physician orders and collaborate with physician.
- Participate in case conferences to discuss team responsibilities, patient progress, plans for continued care, new problems, etc.
- Respond to the physical, mental, social and emotional needs of terminally ill patients and their families; offer ongoing support and palliative care as appropriate.
- Plan patient discharge; recommend referrals for alternative level of care if necessary.
- Recommend to Case Manager needed referrals to other disciplines and/or community resources.
- Chart according to professional and department standards.
- Obtain needed supplies and equipment to provide care.
- Perform patient care responsibilities considering specific needs and standard of care for patient’s age and clinical condition.
- Plan with physician to implement changes to improve patient care.
- Work as part of the interdisciplinary team.
- Report changes in patient’s physical and emotional health to physician.
- Assume role of patient advocate.
- Maintain triage list of patients for winter storm or event that prevents providing services to all scheduled patients.
- Adhere to infection control policies; use PPE.
- Communicate patient data and observations verbally and through documentation.
- Complete all documentation, charting and agency forms in designated time.
