Overview
This nursing position involves providing direct and indirect patient care in a transitional care setting, with a focus on age-appropriate treatment and safety.
Responsibilities- Perform comprehensive assessments, administer medications, manage pain, and develop individualized care plans.
- Communicate effectively with the healthcare team, patients, and families; document and collaborate interdisciplinary.
- Respond quickly to changes in patient condition, assist with procedures, and address psychosocial needs.
- Perform specialized duties including fall risk evaluation, disease management (e.g., COPD, pneumonia, diabetes), infection control, and adherence to safety protocols.
- Current unrestricted RN licensure and BLS certification; rehab nursing experience is preferred.
- Strong communication, organizational, and basic computer skills; cultural sensitivity.
- Ability to follow safety protocols for exposure to blood, pathogens, and hazardous materials.
