As a clinical care coordinator (LVN), you keep our Sherman patients and clinicians in sync. Typically, your day includes:

• Scheduling and coordinating patient visits across the care team
• Reviewing clinical documentation and verifying physician orders
• Communicating with patients, families, and referral sources
• Supporting intake, authorizations, and care-plan updates
• Keeping records accurate, complete, and on time

Moreover, you work closely with nurses and therapists while a supportive office team backs you up. Consequently, you enjoy both structure and variety in the same role.

First, you need an active Texas LVN license. In addition, we look for the following:

• At least one year of clinical or home health experience (coordination or scheduling is a plus)
• Strong organization, communication, and time-management skills
• Comfort with EMR systems and clinical documentation
• A valid Texas driver’s license and reliable transportation
• A caring, patient-first attitude
