Position: Supportive Living Companion

General Description

The Supportive Living Companion works under immediate supervision to provide individually tailored services and activities within the consumer's home and community. The role supports acquiring, retaining, and improving the consumer's self‑help, socialization, and adaptive skills as they relate to active treatment, goals, and objectives. The incumbent reports incidents to appropriate contacts, completes activity logs linked to the person‑centered plan, provides required documentation, and maintains compliance with personnel documents.

Essential Duties and Responsibilities
  • Provide services and activities to the consumer within their home and community as determined by goals and objectives in a person‑centered plan.
  • Train and support the individual in independent living skills, home management skills, cooking, cleaning, socialization, and supported employment in the community.
  • Follow procedures within 24 hours for incidents identified according to Department of Human Services policy.
  • Complete activity logs that occur during the work time frame as identified in the person‑centered plan objectives.
  • Report any changes affecting the individual receiving services as applicable.
  • Ensure the consumer is reminded to take and/or refill medication when necessary.
  • Complete and provide documents as required.
  • Meet initial and ongoing annual training requirements.
  • Maintain compliance with required personnel documents.
  • Perform any other related duties as required or assigned.
Schedule

Friday, 5:00 PM – 8:00 PM;
Saturday/Sunday, 9:00 AM – 5:00 PM.

Education and Experience

High School diploma or GED; one (1) year of relevant, supervised work experience with public health, human services, or other community service agency; or two (2) years of verifiable successful experience working with individuals with developmental disabilities.

Additional Requirements
  • Ability to understand written activity plans, execute instructions, and document services delivered.
  • Ability to communicate effectively with consumers.
  • Ability to access emergency service systems.
  • Ability to access transportation services required as appropriate.
  • Successfully pass a criminal background check, adult and child maltreatment registry checks (with repeat checks as required), and a drug screen prior to employment in accordance with organizational policies and procedures.
Special Entry Rate

CNA certification or Medical Assistant certification qualifies for a special entry rate.

Salary

$12.66 – $13.16 per hour.
