Seeking a licensed Physical Therapist to provide personalized, in-home, and senior living care to older adults.
Responsibilities include conducting evaluations, developing individualized plans, and delivering evidence-based therapy sessions to improve mobility, independence, and quality of life. Therapists will document care accurately, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place. Virtual care options may be incorporated for added flexibility.
Qualifications- Valid Physical Therapy license or eligibility
- Degree from an accredited program
- Basic computer and EMR skills
- Current CPR certification
