Monday‑Friday, Day shift 8:00‑4:30;
No call; no weekends.

Memorial Shiloh Siteman Building #2, Suite 160.

Memorial Hospital Belleville is an acute care hospital offering medical and surgical services plus critical and emergency care including children’s care. The facility provides diagnostic and treatment services as well as heart and vascular care. It is an accredited Chest Pain Center with PCI by the Society of Cardiovascular Patient Care and an Acute Stroke Ready Hospital. Recent accreditation as a designated Lung Cancer Screening Center and Magnet® recognition for nursing excellence support its comprehensive services.

Coordinates the health care of selected patient populations/designated services across the continuum of care. Responsible for coordination of patient contact, patient education, and preparing patients/families for procedures or tests in collaboration with the health‑care team.

• Facilitates patient care by assessing and evaluating patients’ physical, psychosocial and emotional needs.
• Coordinates the delivery of safe and appropriate patient care governed by evidence‑based clinical practice.
• Monitors care and data to optimize patient outcomes.
• Facilitates the education of patients and families; acts as a resource to the health‑care team.
• Ensures actions meet and support the overall goals of the business.
