Job SummaryThe primary responsibility of the Registered Nurse (RN) is to provide comprehensive patient care, including assessing residents, administering medications, monitoring health, coordinating care with other healthcare professionals, and supervising nursing staff.
Essential Duties andJob Responsibilities Patient Assessment and Care Planning
- Conduct thorough assessments of residents' physical, cognitive, and emotional needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, including physicians, therapists, and other healthcare professionals.
- Regularly evaluate and update care plans based on residents' changing needs and responses to interventions.
- Administer medications and treatments as prescribed by physicians, ensuring accuracy and safety.
- Monitor residents' responses to medications and treatments, documenting any adverse effects or changes in condition.
- Monitor and document vital signs, such as temperature, pulse, respiration, and blood pressure.
- Observe residents for any signs of illness or changes in condition and promptly report any concerns to the physician.
- Provide specialized care for residents with wounds, pressure ulcers, or other chronic conditions.
- Collaborate with wound care specialists or other healthcare professionals to ensure optimal wound healing.
- Supervise and provide guidance to licensed practical nurses (LPNs) and certified nursing assistants (CNAs).
- Provide orientation and training for new staff members.
- Maintain accurate and complete documentation of residents' care, including assessments, interventions, and evaluations.
- Communicate effectively with residents, families, and other healthcare professionals.
- Participate in interdisciplinary team meetings to coordinate care and address resident needs.
- Participate in quality improvement initiatives to ensure the delivery of high‑quality care.
- Stay current with best practices and evidence‑based guidelines for nursing care in skilled nursing facilities.
- Must possess a nursing degree from an accredited college or university.
- Must possess a current, unencumbered, active license to practice as an RN in this state (BSN preferred).
- Must maintain an active CPR/BLS certification during employment.
- Prefer experience in long‑term care, geriatric rehab, sub‑acute, or critical care clinical settings.
- Experience in electronic medical records documentation.
