What You’ll Do:

• Deliver top-notch care to post-acute patients Monday through Friday (weekends are for you!).
• Manage patient assessments, care plans, and follow-ups.
• Collaborate with a supportive interdisciplinary team that believes in quality care (and quality coffee).
• Document care with precision because every patient deserves a note-worthy experience.

What You Bring:

• Certification as a Nurse Practitioner (CNP, FNP, APRN) or Physician Assistant (PA).
