Ghost Rx Inc., a Service-Disabled Veteran-Owned Small Business (SDVOSB), is seeking a dedicated Licensed Practical Nurse (LPN) to support primary care services at the Show Low VA Clinic, part of the VA Northern Arizona Health Care System, in Show Low, Arizona. In this full-time, day-shift role you will serve as a key member of the Patient Aligned Care Team (PACT), providing a full range of practical nursing care to the Veterans we are privileged to serve.

Working under the direction of a Registered Nurse or provider, you will perform patient intake, record vital signs, administer medications and treatments, assist with examinations and procedures, and support health-promotion and preventive-care initiatives in an outpatient clinic setting. • Graduation from an approved school of practical or vocational nursing
• Current, full, active, and unrestricted LPN/LVN license in a U.S. state, territory, or compact state
• Six months or more of qualifying LPN experience preferred
• Current Basic Life Support (BLS) certification
• Knowledge of the nursing process and ability to deliver care to patients with varied physical and behavioral needs
• Familiarity with the VHA Patient Aligned Care Team (PACT) model preferred
• Proficiency with electronic health records and clinical documentation
• Ability to recognize and respond appropriately to emergency situations
• U.S. citizenship required for federal facility credentialing
• Ability to pass federal background investigation and meet VA credentialing requirements

• Provide a full range of practical nursing care to Veteran patients in an outpatient primary care clinic setting
• Take and record vital signs including blood pressure, pulse, respiration, pulse oximetry, temperature, pain score, and weight
• Perform routine patient evaluations by obtaining current symptoms, complaints, and history
• Administer prescribed medications and treatments under the direction of an RN or provider
• Assist providers with examinations, procedures, and minor treatments
• Support health-promotion, disease-prevention, and patient-education activities
• Observe, identify, and respond to patient needs, considering emotional, cultural, and age-related factors
• Accurately document patient information and care provided in the electronic health record
• Collaborate with the Patient Aligned Care Team (PACT) to coordinate continuity of care
• Recognize and respond appropriately to emergency situations within the clinical setting
