- With so many opportunities to build processes and improve how we deliver care to our patients, we rely heavily on one-another to communicate changes effectively to ensure seamless support across functions. This often requires this individual to give and receive feedback frequently and communicate challenges or opportunities effectively.
· Provide in-person primary care visits as part of a comprehensive longitudinal care model, working in close collaboration with a virtual PCP who serves as the patient's primary care provider; virtual care modalities may be used as appropriate
· Conduct adult and pediatric wellness visits that address quality care gaps and establish a comprehensive baseline physical exam for all patients
· Provide acute and chronic care requiring physical examination or diagnostic testing, including transition of care visits after high-risk hospitalizations, as needed
· Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care, as needed
· Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists and medical assistants upskilled to support social services navigation
· Document care and maintain clear, comprehensive care plans addressing patient and family needs
· Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance
· Participate in community outreach efforts, as needed
What Do You Bring To The Nest?· Graduate of an accredited Nurse Practitioner or Physician Assistant program with an active, unrestricted Arizona license
· At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred)
· Proven skills, knowledge base, and judgment necessary for independent clinical decision-making and problem-solving
