Registered nurses, regardless of specialty or work setting, utilize the Standards of Professional Nursing Practice and competencies to care for patients at Summit Healthcare. The nursing process, critical thinking and problem-solving skills are used to provide holistic care to individuals, families, groups, communities and populations across the life span. Registered Nurse Seniors are high-performing employees who, in addition to regular RN duties, provide a specialty service or focus within a department or the organization.

This specialty requires additional skills and experience beyond the RN position.

Nursing standards of practice as outlined below:

• Provides initial and ongoing assessment and interpretation of patients within holistic professional nursing practice and patient- and family-centered care.
• Integrates knowledge from environmental factors into the assessment process.
• Assesses and interprets information about the patient’s culture, values, lifestyle, participation in care, and factors impacting outcomes.
• Recognizes the impact of one’s own attitudes, beliefs and values on the assessment process.
• Identifies barriers to effective communication based on psychosocial, literacy, economic, cultural and religious considerations.
• Assesses patient’s ability to manage health to transition care or discharge; identifies needs with patient/family collaboration.
• Identifies and communicates patient or family needs to the interdisciplinary health care team through documentation and vocal communication.
• Provides ongoing monitoring of patient and family response to intervention and identifies deviations from expected outcomes.

• Identifies risks to health and safety including interpersonal, systematic, cultural or environmental lifestyle risks.
• Uses assessment data to identify and communicate actual or potential diagnoses and issues.
• Corroborates diagnoses with patient, family, group or community.
• Prioritizes diagnoses based on mutually agreed goals to meet healthcare needs.
• Documents diagnoses to facilitate outcomes and plans.

• Identifies expected outcomes for an individualized treatment plan in collaboration with the interprofessional team.
• Creates culturally sensitive expected outcomes with patient and care partner to coordinate care.
• Documents expected outcomes as measurable goals.
• Evaluates actual outcomes against expected outcomes.

• Develops, coordinates and updates an individualized plan of care based on evidence, with patient, family and interdisciplinary team to promote continuity of care.
• Develops evidence-based strategies to address diagnoses, problems or issues including health promotion, prevention, healing and supportive care.
• Prioritizes nursing interventions to achieve outcomes and goals.
• Modifies the plan based on ongoing assessment of the patient’s response and other outcomes.
• Develops and coordinates a comprehensive discharge and education plan to meet patient and family needs.

• Establishes and maintains a therapeutic relationship with patient or families.
• Initiates and coordinates interventions consistent with scope of practice and competence.
• Implements prescribed interventions and performs independent nursing actions, including patient and family education.
• Ensures ongoing evaluation of the patient care plan.
• Provides counseling to support skills, social and community supports, health promotion and recovery.
• Collaborates with the team to discuss changes in status and modify interventions.
• Provides information to help patients/families understand health care practices and processes.
• Utilizes educational materials to support understanding of health care and health maintenance.
• Ensures operation of equipment, preventive maintenance, and troubleshooting; maintains equipment inventories; evaluates new equipment and techniques.
