Social Worker
Full-time social worker delivering comprehensive psychosocial services to patients and families, coordinating care plans and resources, and ensuring accurate documentation within the hospital setting.
Key Responsibilities:- Deliver psychosocial assessments, counseling, and resource referrals to patients and families.
- Develop and implement individualized care plans with interdisciplinary teams.
- Advocate for patients' rights and coordinate community resources for continuity of care.
- Maintain accurate documentation in compliance with regulatory standards and hospital policies.
- Participate in ongoing professional development and training opportunities.
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Arizona.
- Minimum of 2 years of clinical social work experience.
- BLS certification.
