Full-Time Hospital Social Worker (LMSW)
This Full-Time Hospital Social Worker (LMSW) will provide comprehensive social work services in a community hospital setting. The role focuses on patient and family support, interdisciplinary collaboration, and discharge planning. Compensation is up to $90,000 per year with a $10,000 sign-on bonus.
Key Responsibilities:- Provide psychosocial assessments, counseling, resource referrals, and discharge planning for patients and families.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Advocate for patients' rights and coordinate community resources to ensure continuity of care.
- Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies.
- Participate in ongoing professional development and training opportunities.
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Arizona.
- BLS certification required.
Skills:
- Minimum of 2 years of clinical social work experience.
