Active LA Medical License (Clean), CDS, DEA (with LA address), Board Eligible/Certified in EM, ACLS, ALS, PALS if not BC in EM, Residency Trained, CME past 2 years, Case logs past 2 years, Copies of all education & training diplomas, ECFMG (if foreign medical graduate), SS Card, Driver's license, Color Photo, TB Results, Flu Vaccine, Health Attestation, Interview w/ Medical Director

576 hospital beds
40.71-hour physician coverage
