This job creates and executes project work plans and revises as appropriate to meet changing needs and requirements in the Program. This job identifies resources needed and manages day‑to‑day operational aspects of the Programs and updates the team on a regular basis. Collaborates with multidisciplinary team and coordinates delivery of plan of care.

EducationRequired - Registered Nurse Diploma
Preferred - Associate's Or Bachelor's Degree In Nursing
Work ExperienceRequired - 5 Years Nursing Experience.
1 year supervisory experience
CertificationsRequired - Current RN License in the state of practice
Basic Life Support (BLS) from the American Heart Association
Preferred - Certification In Clinical Specialty Area

- Must have computer skills and dexterity required for data entry and retrieval of patient information. Must be proficient with Windows-style applications and keyboard.

- Serves as a liaison to regulatory agencies and community partners and participates in meetings, conferences, in‑services, as required.
- Serves as a team lead.
- Chairs committees as assigned.
- Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
- Other related duties as required.
