POSITION TITLE: LPN

DEPARTMENT:
Veterans Division

REPORTS TO:
Program Director

PAY GRADE: TBD

JOB SUMMARY:

The Veterans Division LPN will work with veterans and staff among two residential programs to provide compassionate care to veterans with low to moderate medical complexities with a caseload not to exceed 24 at any given time. LPN will work closely with the Program Director, be a part of the Care Team to include the Overton Brooks VAMC liaisons. The LPN will serve as a critical member of the team and provide relevant healthcare updates regarding the general compliance with medical treatment plans of identified veterans.

The LPN will also consult, on an as‑needed basis, with the direct care staff for general medical and medication information.

ESSENTIAL FUNCTIONS Resident Care & Medical Support
  • Provide direct nursing care to veteran residents within current caseload, including routine health assessments, wound care, and vital sign monitoring.
  • Assist in the development and implementation of individualized care plans.
  • Respond to medical emergencies and administer first aid or CPR as needed.
Medication Management
  • Consult with direct care staff to ensure proper documentation and compliance with medication administration protocols.
  • Educate residents on medication adherence and potential health risks.
Health & Wellness Monitoring
  • Conduct routine health checks and report significant changes in residents’ conditions.
  • Support residents in managing chronic conditions such as diabetes, hypertension, and mental health concerns.
  • Provide basic health education and guidance on wellness practices (Weekly Group).
Coordination & Collaboration
  • Work closely with healthcare providers, case managers, and support staff to address veterans’ medical and psychosocial needs.
  • Assist with coordinating medical appointments, transportation, and follow‑up care.
  • Advocate for veterans’ health and well‑being, ensuring they receive appropriate services and support.
Compliance & Documentation
  • Maintain accurate and timely medical records in compliance with facility policies and regulatory standards.
  • Ensure adherence to infection control protocols and safety procedures.
  • Participate in facility meetings, training, and quality improvement initiatives.
REQUIREMENTS
  • A minimum of five years’ experience working as an LPN.
  • Education above high school diploma or GED.
  • Certification as a Licensed Practical Nurse.
  • Ability to be sensitive and responsive to veteran needs.
  • Ability to communicate effectively with clients, co‑workers, volunteers, and supervisors.
OTHER REQUIREMENTS
  • Must have a valid driver’s license with a good driving record, liability insurance that meets minimal requirements, and vehicle must display a current safety inspection sticker.
  • Criminal background will be checked; must have a cell phone.
