St. Joseph Hospice in Shreveport, LA is seeking a Hospice Admissions RN to assess patients, coordinate care, and educate families.
Schedule:
11am-7pm; travel to patient homes or facilities as needed.
You will work with physicians and the hospice team to implement the Plan of Care, maintain records, and support on-call duties. Louisiana RN license and CPR are required; hospice experience is a plus; ESOP and benefits are provided.
