PositionRN Diabetes Nurse Educator (Diabetes Nurse Educator) – Full-Time, Day shift
Responsibilities- Provide diabetes self-management education to patients with diabetes in classroom and individual settings in accordance with American Diabetes Association Self-Management Education program recognition guidelines.
- Assess patients with diabetes; develop individualized education plans; establish goals and objectives; develop self-management support plans; educate clinical professionals.
- Ensure patient satisfaction, deliver quality care, and maintain compliance with regulatory agency standards.
- Current RN license in the state of Louisiana.
- Preference for Certified Diabetes Educator.
- Ability to attend hospital and department orientation upon hire.
