Social Worker

• Foster a climate, policies, and routines that enable residents to maximize their individuality, independence, and dignity.
• Provide patients with the highest practical level of physical, mental, and psychosocial well‑being and quality of life.
• Identify psychosocial, mental, and emotional needs and assist in developing and providing services to meet those needs.
• Serve in accordance with the National Association of Social Workers (NASW) Code of Ethics and all relevant Federal, State, and Local regulations.

• Bachelor’s Degree in Social Work or Human Services (e.g., Sociology, Special Education, Rehabilitation Counseling, Psychology) from an accredited school.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of two years of nursing experience in a Skilled Nursing Facility (preferred).
