Physical Therapist – Rehab Services
Sidney Health Center is seeking a Rehab Services Staff Physical Therapist to join our I CARE team in the Rehab Department setting. Essential duties include providing a high standard of patient care, including evaluations, progress notes, daily notes, and assessing patients for continued need of care and progressing their plan of care until they can discharge independently.

- Align rehabilitation services with Sidney Health Center’s mission, vision, and strategic goals.
- Perform evaluations, re‑evaluations, and daily physical therapy treatment for a variety of clients.
- Use differential diagnosis to identify impairments, functional limitations, and disabilities and develop an appropriate plan of care.
- Document care in the EHR with appropriate necessity for reimbursement.
- Work with clients in inpatient, outpatient, extended‑care, and specialty rehab services as needed.
- Ensure evidence‑based clinical practice and consistent care standards across service lines.
- Master’s degree or higher in Physical Therapy in the state of Montana.
- Graduation from an accredited professional program.
- Current, unrestricted license in Physical Therapy in the state of Montana.
- Current Basic Life Support (BLS) certification.
