Position Reports toDirector of Pharmacy
SUMMARYCompounds and dispenses prescribed medications, and other pharmaceuticals for patients. Counsels, advises and educates patients, physicians, nurses and other health care workers on the appropriate use of medications to ensure safe utilization of such medications.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Evaluate physician drug orders for appropriateness with regard to patient condition, organ function, disease state, and concomitant therapies to promote optimal patient outcomes. Examples include, collection and interpretation of information regarding patients’ allergies, organ function, drug history, and other information pertinent to drug therapy.
- Process physicians' orders including, but not limited to, order entry into dispensing computer system, appropriate product selection, compounding, labeling and repackaging with completion of appropriate records; delegating product preparation activities to pharmacy technicians when appropriate, within legal limits and hospital policies and procedures.
- Consult with and provide information and guidance to various Medical, Nursing, and other Health Care Professionals concerning drugs and their appropriate use, which may include presenting medical, nursing, or department in-service programs.
- May interact with sick, injured, or disabled patients to determine allergy status, medication history, medication-taking behavior, health status and to educate patients regarding therapy as necessary.
- Respond to cardiopulmonary resuscitation procedures, when requested.
- May have on-call responsibility outside of regularly scheduled work hours.
- Assist in ensuring that drug items are appropriately billed to various areas or patients.
- Assist in ensuring that pharmacy inventory is maintained at appropriate levels.
- Procure non-formulary items after exhausting Formulary alternatives to provide optimal drug therapy.
- Review and assume responsibility for products prepared by pharmacy technicians and assign tasks to pharmacy technicians as necessary to meet workload demands.
- Participate in and/or provide support to hospital committees as agreed upon by committee members and the Director of Pharmacy to help establish drug-related policies, criteria, or to monitor issues related to drugs.
- Monitor and recommend appropriate anticoagulation therapy for clinic patients and respond to nursing staff with therapy changes in a professional and timely manner. Participate in departmental and hospital performance improvement activities to promote quality in the medication use process, including documentation of pharmacist interventions. Assist in insuring that drug storage locations are maintained in an appropriate manner (e.g., floor stock, automated dispensing systems, surgery carts, emergency carts, "crash" carts, etc.).
- Assist in orienting new personnel or students to the departmental services and programs.
- Ensures all controlled substances are purchased, distributed, and documented in compliance with state and federal laws.
- Continually increases knowledge of drugs to perform more effectively as a resource and drug information center.
- Effectively presents information and responds to questions from managers, staff members, patients, patient families, outside agencies, and the general public. This would include the following age groups: pediatrics, adolescent, young-middle aged adult and geriatric.
- Works at maintaining a good working relationship with all patient care personnel and other departments.
- Current license as a Registered Pharmacist required.
