Working Hours: Monday-Friday: 6:00am - 2:30pm / 7:00am - 3:30pm / 9:30am - 6:00pm.
Weekends: 7:00am - 3:30pm.
Shift: VARIES.
Position

Reports to:

Director of Pharmacy.

SUMMARY: Compounds and dispenses prescribed medications, and other pharmaceuticals for patients. Counsels, advises, and educates patients, physicians, nurses and other health care workers on the appropriate use of medications to ensure safe utilization of such medications.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Evaluate physician medication orders for appropriateness based on patient condition, allergies, organ function, disease state, laboratory values, medication history, and concomitant therapies to promote safe and effective patient outcomes.
  • Process physician orders by entering medication orders into the pharmacy computer system, selecting appropriate products, compounding, labeling, repackaging, and maintaining required documentation in accordance with hospital policies and regulatory requirements.
  • Verify the accuracy of medications prepared by Pharmacy Technicians and delegate responsibilities within legal and organizational guidelines to ensure efficient workflow.
  • Provide clinical consultation and drug information to physicians, nurses, and other healthcare professionals regarding medication therapy, dosing, compatibility, interactions, adverse effects, and appropriate drug utilization.
  • Counsel and educate patients and caregivers regarding medication therapy, medication adherence, side effects, and safe medication use as appropriate.
  • Respond to cardiopulmonary resuscitation (Code Blue) and other emergency situations when requested.
  • Participate in the on-call pharmacist rotation as assigned to support pharmacy services outside of regularly scheduled hours.
  • Assist in maintaining appropriate pharmacy inventory levels, including procurement of non-formulary medications when clinically indicated after evaluating formulary alternatives.
  • Ensure medications are accurately charged and billed to patients and appropriate departments.
  • Monitor medication storage throughout the organization, including automated dispensing cabinets, floor stock, emergency carts, anesthesia carts, and other medication storage areas to ensure compliance with regulatory standards.
  • Ensure all controlled substances are purchased, stored, distributed, documented, and reconciled in compliance with state and federal regulations and hospital policies.
  • Monitor anticoagulation therapy for clinic patients, recommend therapy adjustments when appropriate, and communicate recommendations promptly and professionally with nursing staff and providers.
  • Participate in departmental quality improvement initiatives, medication safety activities, and pharmacist intervention documentation to improve the medication‑use process.
