Position: Integrated Hybrid Nurse Practitioner
Location: Sierra Vista, Arizona
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hybrid Role: 1–2 days telehealth (remote) + 3–4 days in patients' homes
Compensation: Base salary +
15% quarterly bonus

We are seeking Nurse Practitioners or Physician Assistants to join a home-based care team serving patients with chronic and complex conditions, including kidney disease. You’ll work alongside a Registered Nurse and Social Worker to deliver personalized, patient-centered care through a mix of in-home visits and telehealth.

• Perform comprehensive assessments and annual wellness visits (home & virtual)
• Develop and manage individualized treatment plans
• Prescribe medications and order diagnostics
• Coordinate care with physicians, specialists, and care team members
• Educate patients and families on disease management and preventive care
• Maintain accurate, timely clinical documentation

• Active NP or PA license and board certification
• Active DEA license
• 2+ years of direct patient care experience
• Ability to practice independently
• Valid driver's license and reliable transportation
