HowYou'll Contribute
This is done in a collaborative multidisciplinary approach embracing quality and safety. Assures quality of care by measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital philosophies and standards of care set by Arizona State Board of Nursing, and other governing agency regulations. Coordinates the delivery of patient care for the Rehab Unit, facilitates problem solving with staff members and communicates necessary issues to nursing administration and the Program Manager.
EssentialFunctions
- Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
- Performs plan of care interventions, including specimen collection and wound care. Administers medication per scope of practice.
- Documents patient care given.
- Assists RN with development of plan of care.
- Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
- Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
- Works as an advocate for the physical and emotional well-being of the patient.
- Adherence to all core measure initiatives, patient safety goals, and NQF Safe Practices.
