Palliative Nurse Practitioner

Overview

Haven Health, the leading Home Health, Hospice, and Palliative Care company in Southern California, is hiring a Nurse Practitioner to join our Palliative Care team. We serve a diverse patient population across Los Angeles and Orange County.

We provide hands‑on training, a strong support system, and an online learning management system for continuing education. iPad and company phone are provided.

• Visit patients in the home and assess their plan of care
• Provide care as a Nurse Practitioner focusing on quality and comfort
• Submit patient records and documentation in a timely manner
• Collaborate as part of the Interdisciplinary Care Team

• Master’s Degree in Nursing
• Licensed Nurse Practitioner in California
• Licensed and insured driver with reliable transportation
• Understanding and acceptance of hospice principles of care
• Strong knowledge of medicine, pharmacology, pain and symptom control, and patient admissions criteria
