Assess patients' physical abilities and determine appropriate treatment plans.
Provide evidence-based physical therapy interventions to improve mobility and function.
Document patient progress and adjust plans based on measurable outcomes.
Adhere to safety, privacy, and regulatory guidelines in all patient interactions.
Educate patients and families on home exercise programs and self-management.
Collaborate with multidisciplinary team to coordinate care and referrals.
Maintain up-to-date clinical documentation in electronic health records.
Meet or exceed productivity targets through effective treatment planning.
Participate in ongoing professional development and contribute to a growth-oriented culture.
Identify patients suitable for additional services and coordinate referrals as needed.
Support pipeline visibility by recording candidate patients and anticipated milestones.
Demonstrate resilience and adaptability in a fast-paced, target-driven environment.
Experience/QualificationsLicensed physical therapist in applicable state with current credentials.
Adhere to safety, privacy, and regulatory guidelines at all times.
Assess patients and create individualized, evidence-based treatment plans.
Deliver PT interventions to improve mobility, strength, and function.
Document progress and update plans with measurable outcomes in EHR.
Educate patients/families on home exercise programs and self-management.
Collaborate with multidisciplinary team; coordinate referrals as needed.
Identify patients for additional services; manage referral pipeline.
Maintain productivity targets through effective treatment planning.
