Provide compassionate assistance with daily living activities, support mobility and transfers, aid with meals and personal care, and monitor comfort and safety while documenting observations for the care plan. Collaborate with a skilled interdisciplinary team, follow individualized care plans, maintain the highest standards of safety and infection control, and communicate clearly to ensure consistent, respectful care.
