• Administer prescribed medications and treatments according to physician orders and facility policy
• Monitor residents’ conditions, perform routine assessments, document vital signs, and report changes to the supervising RN
• Provide direct patient care including activities of daily living, wound care, and assistance with rehabilitation as needed
• Support the care plan by collaborating with the interdisciplinary team to address resident needs and goals
• Ensure safe patient transfers, alignment with mobility plans, fall prevention strategies, and infection control practices
• Document care accurately in the resident record and maintain privacy and compliance with regulatory requirements

• Active Licensed Practical Nurse license in New Mexico
• Diploma or certificate from an accredited practical nursing program
• Minimum experience in a long‑term care or skilled nursing setting preferred
• Ability to work 12-hour daytime shifts and meet guaranteed weekly hours
• Strong communication skills, teamwork orientation, and ability to adapt in a fast‑paced LTC environment
