LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting. Ensure compliance with safety standards and deliver compassionate patient support under supervision.

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document residents’ health status and report any changes.
• Follow facility policies regarding infection control and safety protocols.
• Assist residents with activities of daily living to promote well-being.
• Communicate effectively with interdisciplinary team members to coordinate care.
• Maintain accurate and detailed records of patient care activities.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience in LTC or SNF environments preferred.
• Ability to follow detailed care plans and protocols.
• Strong communication and interpersonal skills.
• Demonstrate professionalism and reliability.
• Must have current immunizations, including COVID-19 vaccines, or valid exemptions.
