LVN ​/ LPN – LTC​/SNF

LVN/LPN Position in Long-Term Care or Skilled Nursing Facilities

Assist in providing quality patient care within long-term care or skilled nursing facilities as an LVN or LPN. Ensure that patient needs are met with professionalism and adherence to healthcare protocols.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Document patient information and care provided in compliance with facility policies.
• Monitor patient conditions and report any changes to the healthcare team promptly.
• Assist with patient assessments, wound care, and other clinical tasks.
• Follow infection control procedures, including COVID-19 protocols.
• Collaborate with interdisciplinary team members to coordinate patient care.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Current CPR and BLS certification.
• Proven experience in long-term care or skilled nursing settings.
• Ability to adhere to strict infection control and safety protocols.
• Strong communication and documentation skills.
• Must bring valid photo identification and be able to wear a photo badge at all times.
