LVN ​/ LPN – LTC​/SNF

Nursing Position in Skilled Nursing Or Long-Term Care Facility

Provide high-quality nursing care to residents in a skilled nursing or long-term care facility. Support patient well-being through medication administration, patient assessments, and collaboration with the healthcare team.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Conduct patient assessments to monitor health status and report changes.
• Maintain detailed and compliant documentation of care provided.
• Collaborate with registered nurses and other healthcare professionals to develop care plans.
• Ensure adherence to safety protocols and infection control standards.
• Educate patients and their families on health conditions and care plans.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC or SNF settings preferred.
• Ability to perform patient assessments and administer medications competently.
• Excellent communication and teamwork skills.
• Demonstrate attention to detail and adherence to regulatory standards.
• BLS certification required; additional CNA or geriatric care experience is a plus.
