LVN​/LPN - LTC​/SNF

Job Summary:

Provide high-quality licensed practical nurse (LPN) care in a skilled nursing facility, ensuring patient safety and comfort. The role involves collaborating with the healthcare team to deliver comprehensive nursing services to the residents.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and assess patient conditions, reporting changes to the nursing supervisor.
• Document patient care activities thoroughly and in compliance with facility policies.
• Assist with ADLs (Activities of Daily Living) and provide wound care as needed.
• Maintain a safe and clean environment, adhering to infection control protocols.
• Communicate effectively with residents, families, and team members to coordinate care.

Qualifications:

• Hold a valid state license as an LVN or LPN.
• Demonstrate prior experience in long-term care or skilled nursing facilities.
• Possess current CPR and First Aid certification.
• Exhibit strong communication and interpersonal skills.
• Ability to work a flexible schedule, including weekends.
• Valid state healthcare-institutional immunization documentation, including COVID-19 vaccination or approved exemption.
