LVN​/LPN – LTC​/SNF

Job Summary:

Provide licensed practical nursing care in a skilled nursing facility, ensuring patient comfort, safety, and well-being. Administer medications, monitor patient conditions, and collaborate with healthcare teams to support recovery and health maintenance.

Responsibilities:

• Perform comprehensive patient assessments and document clinical observations.
• Administer medications and treatments according to physician orders and facility protocols.
• Monitor patient vital signs and respond promptly to any changes in condition.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Educate patients and their families on care procedures and health management.

Qualifications:

• Hold an active LVN or LPN license valid in the state of practice.
• Have recent experience working in a LTC or SNF setting.
• Demonstrate excellent patient care and communication skills.
• Be able to multitask efficiently in a fast-paced environment.
• Complete a criminal background check and provide up-to-date immunization records, including COVID-19 vaccination or approved exemption.
