LVN​/LPN – LTC​/SNF

Job Summary:

Provide comprehensive nursing care to residents in long-term care or skilled nursing facilities. Ensure patient safety, comfort, and quality care while adhering to regulatory standards and facility protocols.

Responsibilities:

• Administer medication and treatments accurately according to physician orders.
• Monitor residents’ health status and record observations meticulously.
• Collaborate with healthcare team to develop and implement individualized care plans.
• Educate patients and family members about health conditions and care procedures.
• Maintain timely documentation of all nursing activities and interventions.
• Ensure compliance with infection control policies and safety regulations.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Certified in CPR and Basic Life Support (BLS).
• Proven experience working in LTC or SNF settings preferred.
• Able to follow detailed instructions and demonstrate strong clinical skills.
• Able to effectively communicate with patients, families, and team members.
• Must bring a valid government-issued  wear a photo badge at all times.
