Job Summary

The NN team is responsible for collaborating with various stakeholders, including local social services and healthcare providers, mental health and substance use treatment facilities, legal agencies and professionals, housing department, and faith-based leaders to identify and link members of the community looking to achieve wellness in their neighborhoods to NN locations. The Registered Nurse is paired with Community Health Workers, delivers comprehensive care directly in homes and neighborhood hubs, such as schools, libraries, senior centers, apartment buildings, faith-based organizations, and other identified pop-up locations.

Duties

and Responsibilities

The primary responsibilities of this position are:

Administration
  • Participation in local/state meetings, conferences, community meetings
  • Collaborate, communicate and coordinate follow up care with Neighborhood Nursing team.
Case Management
  • Meet with individuals and families in a variety of community settings including homes, community centers, libraries and other “hub” sites.
  • Link and enroll participants into community resources including health insurance, primary care, specialty care, behavioral health, housing, transportation, food support and utility support
  • Effectively use motivational interviewing techniques to engage and coordinate care/services to clients.
  • Provide client centered services in a trauma informed approach
  • Prioritize acute (urgent) and primary/secondary prevention health and social needs.
  • Provide Point of Care (POC) testing (e.g. blood pressure, blood glucose), health screenings and testing, vaccinations, health education and other activities, in groups and individually.
  • Engage participants in problem solving to identify behavioral and other environmental contributors that impacts their health (physical, emotional and social).
  • Provide mental health and substance disorder psychoeducation in a group or individual setting.
  • Develop and provide health education and training materials for clients and community members.
Documentation
  • Create and document client treatment plans, interventions and recommendations, screening results and health risk assessments
  • Timely data input and documentation in electronic health records and other data management systems.
  • Document and record outcomes of referrals and appointments
  • Create and produce reports on demand
  • Develop and provide health education and training materials for clients and community members.
  • Initiate and assist in developing strong community relationships, attend community meetings and other activities that cultivate trust.
  • Function as a Team Leader to the Community Health Worker team
Human Resources
  • Timely and accurately submit timesheets.

QUALIFICATIONS
  • Bachelors in Nursing required, Active State of Maryland Registered Nurse License
  • At least two years of experience providing home/community outreach to a diverse client population
  • Demonstrated knowledge in the following areas:
    Community Outreach, Maternal and Child Health Disparities
  • Demonstrated knowledge and experience working with Health Choice Medicaid recipients and Managed Care Organizations.
KNOWLEDGE, SKILLS & ABILITIES
  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365).
  • Organizational skills with the ability to meet a demanding workload
  • Detail oriented to complete requirements of files and contract compliance.
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • Excellent oral and written interpersonal and communication skills required
  • Proven ability to work efficiently and meet deadlines
