We are hiring an experienced Home Health Occupational Therapist (OT) to provide patient-centered therapy in the home setting. This position offers autonomy, flexible scheduling, and strong interdisciplinary collaboration. If you value independence, meaningful patient relationships, and the ability to see real progress in your patients this is for you.
Key Responsibilities- Complete in-home OT evaluations and reassessments
- Develop individualized treatment plans
- Deliver skilled interventions to improve ADLs and functional independence
- Educate patients and caregivers on safety and home exercise programs
- Track progress and adjust care plans accordingly
- Document visits accurately and timely in EMR
- Collaborate with physicians, nurses, and care team members
- Supervise Certified Occupational Therapy Assistants (COTAs)
- Maintain Medicare, HIPAA, and agency compliance
- Graduate of an accredited Occupational Therapy program
- Active Maryland OT license (required)
- Minimum 3 years clinical experience (home health preferred)
- Strong assessment and care planning skills
- EMR experience preferred (Well Sky/Kinnser or Homecare Homebase)
- Valid driver’s license and reliable transportation
