Recovery Coach – Behavioral Health & Wellness

• Serve as a personal guide and liaison for individuals with substance use disorders
• Support patients by assisting them with identifying treatment needs
• Develop, maintain, and monitor individualized treatment plans
• Provide outreach, support, and recovery check-ups on site in treatment settings
• Link patients to available resources including education, safe housing, and recovery supports
• Assist patients in crisis situations involving social, emotional, and health problems

• High School Diploma or equivalent
• Peer Recovery Certification issued by state board
• State criminal background check and Federal (if applicable)
• Valid Driver’s License (preferred)
• Certificate of completion of the CCAR Recovery Coaching in the Emergency Department (preferred)
• Active participation in recovery for two or more years and current active attendance at community recovery meetings (preferred)

Demonstrates expertise in developing and monitoring individualized treatment plans for individuals with substance use disorders, while providing essential support and resources to facilitate recovery. Strong background in peer recovery and active participation in community recovery initiatives enhances the ability to assist patients effectively.
