LMSW Case Manager, Social Work

Kaiser Permanente is seeking an LMSW Case Manager to provide inpatient social work services within our technology-enabled care environment. In this role, you will conduct psychosocial assessments, coordinate discharge planning, and connect patients to community resources using our integrated digital platforms. You will collaborate with physicians, nurses, and care teams, document in electronic health records, and help optimize patient flow. Responsibilities

• Conduct comprehensive psychosocial and risk assessments for inpatient populations
• Develop and implement individualized discharge and care plans
• Coordinate services with physicians, nurses, and interdisciplinary care teams
• Utilize electronic health records and digital tools to document care and track outcomes
• Link patients and families to community resources and support services
• Provide crisis intervention and brief counseling as appropriate
• Collaborate with utilization review and care coordination teams to support appropriate length of stay
• Educate patients and families on care plans, benefits, and available support programs
• Participate in quality improvement initiatives and process optimization
• Maintain compliance with regulatory, ethical, and documentation standards
