Maryland Care Management, Inc. (MCMI) seeks a Social Determinants of Health (SDoH) Care Coordinator to join the MPC team. This role serves members in both clinical and community settings, addressing social barriers and connecting them to needed resources and support.
You will conduct intake and SDOH assessments, develop care plans, and partner with community organizations. Travel 50-75% across Central Maryland is required, with a focus on improving health outcomes and coordination of care.
