Join our wonderful team as a Hospital Liaison today! The Autumn Lake Healthcare is hiring an experienced and motivated Hospital Liaison to join our team! The Hospital Liaison will play an important role in building strong relationships with hospitals, healthcare providers, patients, and families while helping ensure a smooth transition into our skilled nursing facilities.

Responsibilities for Hospital Liaison:
  • Develop and maintain strong relationships with hospitals, discharge planners, case managers, and other healthcare professionals
  • Serve as a key point of contact for hospital referrals and potential admissions
  • Assist with coordinating a smooth transition from the hospital to the appropriate skilled nursing facility
  • Educate referral sources, patients, and families on available services and care options
  • Collaborate with facility leadership and admissions teams to support census growth and referral development
  • Maintain consistent communication and follow-up with referral partners
Qualifications & Experience Requirements for Hospital Liaison:
  • Previous experience as a Hospital Liaison preferred
  • Prior healthcare experience required
  • Strong communication and relationship-building skills
  • Ability to work independently while collaborating with multiple facilities and healthcare partners
  • Strong organizational and follow-up skills
  • Experience in skilled nursing, long-term care, hospital relations, admissions, or healthcare marketing preferred
