This Behavioral Health Intake Care Coordinator I works closely with the Behavioral Health Intake Team to respond to the needs of participants seeking services at Mary’s Center behavioral health clinics. This position is responsible for supporting participants in navigating the Open Access Intake system and in following up with participants after their initial intake screenings. In addition, responsibilities will include coordination and scheduling of initial behavioral health appointments, linkages to other departments within Mary’s Center, and assisting with referrals to external resources.

Reportability

This position will report directly to the Behavioral Health Intake Manager.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

  • Support access to Open Access Intake windows by reaching out to participants referred for behavioral health services, assessing interest, and providing information on the Open Access Intake schedule.
  • Support Open Access Intake system by gathering preliminary intake information, completing initial screeners, and providing information about services and policies to new participants.
  • Maintain up-to-date documentation of all outreach and assessment activities.
  • Participate in weekly clinical case conference to triage acute cases and facilitate case assignments across all sites with Intake Manager and Intake Therapist.
  • Coordinate scheduling of groups for participants on the waiting list.
  • Facilitate scheduling of initial behavioral health appointments, including reminder calls and follow-up tasks as needed.
  • Facilitate linkage to other Mary’s Center departments, programs, and services.
  • Facilitate linkage to external resources as needed.
  • Connect with participants stepping down from individual therapy services about group options and external resources.
  • Respond to client concerns and provide support in troubleshooting barriers to accessing care.
  • Develop knowledge of other providers, agencies, and higher levels of care to appropriately refer participants when at capacity or Mary’s Center cannot serve a participant’s needs.
  • Perform other duties as assigned.
Qualifications

Knowledge, Skills & Abilities –
  • Bachelor’s degree OR High School Diploma with 1 or more years of relevant work experience.
  • Thorough understanding of privacy and confidentiality regulations. Familiarity with behavioral health preferred. Knowledge of community resources preferred.
  • Organization, documentation, and record-keeping.
  • Ability to organize and prioritize daily tasks; ability to stay calm under pressure and in face of emotional distress; ability to work one’s own and with a team.
Language Requirement –

Must have the ability to communicate effectively in English and Spanish as used by participants.

Physical Demands –

Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear.

Work Environment

Mostly in a typical office or clinic setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions. Opportunity to work a hybrid schedule – remotely & onsite, as needed.
