Main Purpose of Job

The main purpose of this position is to implement case management services as part of Somerset County's multiagency Mobile Integrated Health (MIH) care team and Cancer Screening Programs. This position will advocate on behalf of clients and work directly with them to identify challenges that may affect their health outcomes. This position will assist clients in addressing identified challenges to improve health outcomes and reduce reliance on emergency department services.

This position is emergency essential.

MINIMUM QUALIFICATIONS

Education:

Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience:

One year of administrative staff or professional work in a health care setting. Notes:
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess the following:

• Experience providing case management services.
• Experience coordinating services and connecting clients to community resources.
• Experience working with cancer screening programs.
• Experience maintaining client records and entering program data accurately and timely.
• Experience using Microsoft Office and Google Workspace applications.
