PT - Physical Therapist

Montgomery County, MD

This is a 13-week contract position.

Day shift, Monday to Friday (08:00 - 17:00).

We are seeking a dedicated and skilled Physical Therapist to join our home health team. The ideal candidate will be responsible for providing high-quality care to adult patients in their homes. This position requires a compassionate approach and the ability to work independently while ensuring the well-being of our patients.

• Conduct patient assessments and evaluations to determine care needs.
• Develop and implement personalized treatment plans.
• Perform therapy interventions to improve patient mobility and functionality.
• Educate patients and their families about home exercise programs and therapy techniques.
• Maintain accurate and timely documentation of patient progress and therapy outcomes.
• Collaborate with interdisciplinary teams to optimize patient care.

• Valid Maryland PT State License required.
• Current BLS certification is mandatory.
• Minimum of 1 year of experience in a related field, preferably in home health.
• Experience with adult health care and patient management is essential.
• Two supervisor references required.
• Candidates must be nicotine-free.
