You’ll work with a diverse home health population including:
- Orthopedic conditions
- Neurological disorders
- Post-surgical recovery
- General medical conditions
- Fall prevention and home safety
- Functional decline
- Activities of daily living (ADL) deficits
As a Home Health Occupational Therapist, you’ll help patients regain independence and safely perform everyday activities in their own homes.
Responsibilities include:
- Evaluate and treat patients in their home environment
- Develop individualized, evidence-based treatment plans
- Improve performance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Assess home safety and recommend adaptive equipment
- Educate patients and caregivers on treatment plans and fall prevention
- Document care using EMR
- Collaborate with physicians, nurses, therapists, and case managers to optimize patient outcomes
- Graduate of an ACOTE-accredited Occupational Therapy program
- Current Maryland Occupational Therapy license (or eligibility)
- American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification
- Strong communication, organization, and time management skills
- One year of home health experience
- Inpatient rehabilitation or skilled nursing experience
- Experience with EMR documentation
- Ability to work independently while managing a mobile caseload
