Registered Nurse (RN)to provide skilled in-home nursing services throughout Montgomery County and Prince George’s County, Maryland.

- Perform comprehensive patient assessments in the home setting
- Develop and implement individualized care plans
- Provide skilled nursing care including wound care, medication management, and disease education
- Complete accurate and timely OASIS documentation
- Educate patients and caregivers on disease management and safety
- Coordinate care with physicians and interdisciplinary team members
- Monitor patient progress and adjust care plans as needed
- Active Maryland Registered Nurse (RN) license
- Minimum 2 years nursing experience (home health preferred)
- OASIS experience preferred
- EMR experience (Well Sky/Kinnser or Homecare Homebase a plus)
- Strong assessment and documentation skills
- Valid driver’s license & reliable transportation
