American Traveler is seeking an experienced RN Case Manager for a 13-week travel contract in Silver Spring, MD, requiring acute care nursing experience and knowledge of health care reimbursement systems.

Job Details

• Acute inpatient hospital setting focusing on care coordination and discharge planning,

• Collaborates with physicians, nursing, utilization review, and other healthcare team members to support care progression and safe transitions,

• Caseload includes acute inpatient members requiring discharge needs assessment, post-acute referrals, and transition of care coordination,

• Post-acute coordination includes Home Health, DME, IV Infusion, SNF Sub-Acute, and Acute Rehab,

• Day shift: 8:30 AM - 5:00 PM (8 hrs/day, 40 hrs/week),

• EMR:
Epic,

Job Requirements

• Active MD or Compact RN license required; no pending licenses accepted,

• All certifications must be active at time of consideration,

• Associate's Degree in Nursing (ADN) required,

• Minimum 1 year of experience in acute medical/surgical, ED, or critical care nursing,

• Experience using an electronic medical record system required,

• Knowledge of health care reimbursement systems required, including Medicare Advantage, Commercial payer, Medicaid, and CMS regulatory rules,

• Last 4 digits of SSN and date of birth required for consideration,

Additional Information

• Conducts initial face-to-face assessments for every admitted patient within 24 hours of admission to identify discharge needs,

• Documents and communicates assessment outcomes and transition plans in accordance with discharge planning policies and regulatory requirements,

• Coordinates referrals to in-network vendors for post-acute levels of care,

• Collaborates with community providers and ambulatory case managers to facilitate appropriate level of care transitions and prevent readmissions,

• Local candidates are accepted; local and travel rates are the same,

• Travelers who have previously worked at this health system may not exceed two cumulative years of assignment without a 90-day consecutive break
