Travel RN - Case Management​/Utilization Review - Case Management

American Traveler is hiring a travel RN Case Manager for an acute inpatient care coordination role in Silver Spring, MD, requiring an active MD or Compact RN license and Epic experience.

Job Details

• Acute inpatient hospital setting focused on discharge planning and care coordination,

• Collaborates with hospital-based physicians, nursing staff, utilization review, and ancillary team members,

• Works with patients, families, and community providers to develop and coordinate safe discharge plans,

• Coordinates transitions of care with community providers and ambulatory case managers to prevent readmissions,

• Day shift: 8:30 AM - 5:00 PM (8-hour shifts),

• 40 hours per week,

• Uses Epic EMR,

• 14-week contract,

Job Requirements

• Active RN license - Maryland or Compact license required and must be in hand at time of consideration (pending licenses not accepted),

• Active certifications required at time of consideration (no pending certifications accepted),

• Experience in case management, discharge planning, or care coordination in an acute inpatient setting,

• Epic EMR experience,

• Last 4 digits of SSN and date of birth required for consideration,

Additional Information

• Responsible for overseeing the management and coordination of care for the acute inpatient population, including facilitating appropriate level-of-care transitions and discharge planning,

• Collaborates with rounding physicians, patients and families, nursing, and utilization review to ensure clinically appropriate and cost-effective care progression,

• Contingent workers are subject to a tenure policy limiting assignment time to no more than two years (including assignments with lapses of less than three months) before a mandatory 90-day break is required
