Home Health LPN delivering skilled nursing care to clients in accordance with the plan of care prescribed by the physician and assigned by the supervising registered nurse.

• Deliver skilled nursing care to clients as per the plan of care.
• Maintain patient documentation using the Epic EHR System.
• Establish and maintain meaningful 1:1 patient relationships.
• Operate within a home health setting without on‑call requirements.

• Pay Range: $26.07-$41.71 per hour.

• Min. one year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the State of Maryland.
• CPR certification required.
• Passed NAPNES or equivalent medication exam before administering medications.
• Current driver’s license and reliable transportation.
