RN - Home Health

Day shift, Monday through Friday, 8:00 AM - 5:00 PM

We are seeking a skilled Registered Nurse (RN) to join our Home Health team. The RN will be responsible for managing patient care within the home setting, ensuring the delivery of high-quality nursing services. This position involves conducting assessments, planning care, and coordinating services for adult and geriatric patients discharged from hospitals, medical offices, and skilled nursing facilities.

• Conduct patient assessments and develop individualized care plans.
• Provide medication administration and management.
• Perform clinical assessments and documentation via Home Care Home Base (HCHB).
• Coordinate and manage patient services ensuring compliance with care plans.
• Educate patients and families on health conditions and care plans.
• Monitor and document changes in patient conditions.

• Active RN license in the state of Maryland.
• Minimum of 1 year of home health nursing experience required.
• Experience using Home Care Home Base (HCHB) is required.
• BLS certification is required.
